Work Schedule
  • Night shift: 6 p.m. – 6:30 a.m.: 48 hours / 2 weeks
  • Every third weekend
  • Every other holiday rotation
Pay
  • LPN Pay: $22.20 – $32.00 per hour.
  • RN Pay: $34.00 – $51.00 per hour.

Job Description Summary

Delivers nursing care by developing a nursing diagnosis; establishing nursing care objectives; defining nursing interventions; providing physical, emotional, psychological, and spiritual support to residents, families, and friends to understand residents' conditions, medications, and self‑care skills; noting changing conditions and needs; re‑evaluating care objectives.

RN Required Education, License/Certification, or Work Experience
  • Registered Nurse (RN) – Board of Nursing
  • An active license in the state of practice (Upon Hire)
RN Preferred Education, License/Certification, or Work Experience
  • 1–3 years’ experience working in a clinical or long‑term care environment.
What you will do
  • Perform all licensed practical nursing functions and services as directed by the Registered Nurse to all residents.
  • Complete assessments on residents, make appropriate interventions, apply knowledge, prioritize resident care, and use the nursing process (assess, plan, implement, evaluate).
  • Comply with all pharmaceutical policies and procedures, ensure that all medications/treatments are given safely and accurately in accordance with the physician’s orders.
  • Administer medications according to facility policy, competent in Pyxis medication administration system.
  • Participate in the care‑planning process and assure that resident care is implemented consistently and appropriately to the age of those served.
  • Initiate and update the plan of care as appropriate.
  • Comply with standards of professional documentation.
  • Communicate and manage the delivery of resident care, ensuring quality care is provided.
  • Guide staff in meeting the needs of the residents, practicing routines based on residents’ needs.
  • Participate in physician rounds, communicate changes in resident condition to physician, utilize SBAR communication tool.
  • Process and transcribe physician’s orders and implement according to facility policy.
  • Seek educational opportunities, share knowledge with others, assist with orientation of new staff.
  • Investigate incidents, patient/family complaints and report immediately to VP of Resident Care or Administrator on‑call.
  • Facilitate conflict management while respecting diversity of people, their strengths and ideas.

  • The individual must be able to work the hours specified.
  • Must perform each essential job function satisfactorily, including visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds.

LPN Required Education, License/Certification, or Work Experience
  • Licensed Practical Nurse (LPN) – Board of Nursing
  • An active license in the state of practice (Upon Hire)
LPN Preferred Education, License/Certification, or Work Experience
  • Less than 1 year experience in Long Term Care.
