Registered Nurse (RN) Care Team Case Manager

The Registered Nurse (RN) Care Team Case Manager (CTCM) provides services in support of medical providers and the clinical team. Within scope of licensure and responsibilities, the RN CTCM provides care coordination, case management, and acute interventions following established protocols. This position also manages patients through transitions of care and the care continuum. This opportunity is full-time, 10-hour or 8-hour shifts scheduled on weekdays, and located in our clinic in Stevenson, Washington.
Minimum Qualifications- Graduate of an accredited nursing program
- Registered Nurse with current Washington license and ability to obtain within 3 months of start date.
- Experience with adult and pediatric primary care
- Current CPR/Basic Life Support (BLS) certification
- Valid driver’s license and proof of auto insurance.
- Strong written and oral communication skills.
- Able to communicate clinical concepts in both oral and written form to clinical and non-clinical audiences.
- Able to prioritize and organize work according to multiple and, at times, competing timelines
- Able to meet timelines for team and organizational goals safely and with high level of quality.
- Able to inspire trust and confidence, and communicate effectively with internal and external audiences
- High level computer skills, including use of Microsoft Office applications:
Word, Excel, Outlook, Epic, and other systems - Knowledge of and compliance with HIPAA regulations
- Knowledge of customer service and workplace safety
- React and remain calm and effective in high pressure and emergency situations
- Able to perform or operate medical equipment, venipuncture, and IV skills
- Experience at a FQHC or look alike.
- Experience working in an integrated team-based setting.
- Experience with EPIC Electronic Medical Records.
- Familiarity with a variety of healthcare concepts, practices, and procedures.
- Experience as a supervisor or group leader in health care or other business.
