Job Summary:
Registered Nurses (RNs) are part of our Integrated Clinical Care team. They coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their families. The RNs provide excellent health education, nursing care, and case management services that enhance the quality of life of the PGST Community Health Center's patients.
Duties Include:
The specific areas of focus for the duties below may be assigned periodically by the Lead ARNP.General Nursing:
Review and may order laboratory tests, immunizations, diagnostic procedures, and consultation services in conjunction with the patient’s primary care provider, following the PGST Community Health Center care standards and provider standing orders.
Interpret laboratory findings and initiate action for necessary care (such as scheduling follow-up appointments with health care providers).Notify patients of lab results/diagnostic procedure results as requested by health care providers.
Assess the immunization status of patients and their family members and recommend vaccinations.
Administer immunizations following the clinic’s standing orders for adult and childhood vaccinations.
Order over-the-counter medications when indicated, in conjunction with the patient’s primary care provider.
Perform procedures in the community setting that may be therapeutic and helpful to assess the patient’s immediate health status (e.g., weight, height, abdominal circumference, and BMI measurements); vital signs, including temperature, pulse, and respirations; administer vaccinations; monitor blood pressure; manage and administer medications; and monitor blood glucose.
Assist in working with the providers as needed.
Inputs historical medical data for all patients into the electronic medical record and Child Profile Immunization Data system.
Advise medical staff of prevention interventions needed and provide prevention interventions as prescribed.
Complete chart reviews of medical records and input historical data into the patient's electronic medical record and Child Profile Immunization registry.
Assess and perform prevention interventions based on the patient’s sex, age, and chronic disease history (e.g., immunizations, referrals for age/sex/disease-related screenings, lab work for prevention screenings, spirometry and peak flow testing, etc.).Notify patients of necessary exams and prevention interventions, schedule appointments, and complete referrals to ensure the completion of needed prevention interventions.
Interview patients to obtain information for the health history and the health summary documentation system; measure and record the patient's vital signs (blood pressure, temperature, pulse, respiration), height and weight, and basic vision screening when asked to assist with check-in for provider visits.
Physically assess patients
Communicate with medical and behavioral health providers utilizing the SBAR format (Situation, Background, Assessment, Recommendations)
Performs procedures such as immunizations, medication injections, wound care, phlebotomy, etc.
Instruct and counsel patients regarding prescribed treatments and regimens per the providers’ instructions.
Performs and processes routine laboratory tests and coordinates with the lab to obtain test results as needed.
Sterilizes and services clinic equipment and lab areas per established procedures.
Assists the providers during examinations and procedures by setting out equipment and performing other assigned tasks.
Monitors supplies to ensure a consistent stock; orders clinic supplies, immunizations, and pharmaceuticals from established sources per approval of the clinic coordinator. Conducts regular inventory of medical, pharmaceutical, janitorial, and other clinic supplies.
Maintains the highest standards of ethical conduct, including maintaining the strict confidentiality of patient information.
Ensures the timely, accurate communication of issues to the clinic medical providers, patients, the community, staff, and local medical providers by expressing self clearly and effectively when talking to groups and individuals.
