Part Time Clinician (Nurse Practitioner or Physician Assistant)
OverviewSignify Health, part of CVS Health, seeks a clinician to提供 In-Home Health Evaluations, engaging with patients in their homes and helping bridge gaps in care. The role involves building trusted relationships and delivering flexible, comprehensive and personalized health evaluations at patients’ front doors.

- Help visit millions of people nationwide, aiming to connect them with the right care for their unique needs.
- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits as required.
- Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (spirometry, diabetic eye exams, bone density screening, etc.).
Preferred Qualifications
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Multi‑state licenses (preferred).
- Ability to cover the following primary locations:
Vancouver, WA and surrounding area.

Pay Range: $46.03 – $99.14 per hour. The actual base salary offer will depend on experience, education, geography and other factors.
