The Department of Corrections is seeking a highly motivated and qualified Registered Nurse 3 for the Airway Heights Corrections Center (AHCC) located in Airway Heights, WA.

This position supports the Department's mission to improve public safety by delivering and supervising others in the delivery of nursing services that promote health, foster healing, and support coping and adjusting to various health conditions. This position supports the mission of the Health Services Division by providing necessary, comprehensive, quality health services and promoting incarcerated individual wellness.

Ensure continuity and coordination of care for patients identified as medically complex:

• Collaborates with Reentry Nurse and Psych Social Worker to ensure continuity of care for high complexity patients after release, to include clinical communication and health information transfer with the patient's community PCP, ensuring post-release appointments in the community are made, and post-release self-management plan is in place.
• Ensures continuity of care at inta-system transfer by direct communication with the receiving facilities primary care team and/or care coordinator as applicable.
• Ensure continuity of care after hospital discharge or emergency department evaluation.

Provides direct patient care and monitoring to the facility's high complexity patient population:

• Performs initial comprehensive nursing assessments on patients newly referred for care management, including collection and review of subjective and objective data.
• Evaluates patients post discharge from hospitalization or emergency department evaluation.

Develops and implements a care plan for a facility's identified high complexity patient population in collaboration with primary care team:

• Synthesizes available objective and subjective medical data to create a person and condition specific plan of care upon initial evaluation.

Promotes patient self-management and provides education:

• Promotes patient self-management and empowers patients to achieve maximum levels of wellness and independence

Collaborates with primary care team, other clinical disciplines, and HS administrative staff to care for the facilities identified high complexity patient population:

• Receives and evaluates referrals from primary care team and other Health Services staff for enrollment in the DOC HS Care Management program.
• Works in conjunction with Patient Services Representative (PSR), Patient Navigator, and primary care Medical Assistant to ensure completion and follow up of off site referrals.
• Collaborates with primary care practitioner in management of patient's chronic and acute illness, including communicating any changes in the patient's status.
• Collaborates with other clinical disciplines as indicated including behavioral health, dental and SARU.
• Participates in scheduled multidisciplinary team meetings with the primary care team, Facility Medical Director and other clinical disciplines to review the status of the complex patient population as needed.

Participates in system-wide development of DOC HS Care Management program:

• Participates with other Health Services Staff in periodic system-wide HS Care Management program development meetings, via tele-conference or in-person.

Other duties as assigned:

• Established therapeutic relationships that preserve professional boundaries with patients.

Required qualifications:

• Initial and continuous unencumbered licensure in the State of Washington as a Registered Nurse. OR An unencumbered Multi-State License from a participating Compact Licensure State and completion of the MLS demographic survey. AND Two years' experience working as a Registered Nurse. Training by the American Heart Association in Healthcare Providers CPR. Proficient using computer-based programs to schedule, monitor and track patient information, perform data entry and word processing, and communicate via email, as well as other programs that may be needed to complete assigned work.
