RN Case Manager - Drive Time

We are seeking experienced RN-Case Managers to provide in-home care for former Nuclear Weapons Workers with chronic or terminal illnesses caused by occupational exposure.

• Responsibilities include conducting weekly in-home nursing visits, performing physical assessments, ensuring home safety, managing medications, and overseeing caregiving staff.
• Coordinate and evaluate patient care, collaborate with team members, and serve as a clinical resource to ensure quality services.
• Participate in staffing, training, documentation review, and quality improvement initiatives, maintaining compliance with agency policies and scope of practice.
• Assist with client and physician engagements and support agency operations as needed.

• Graduate of an accredited nursing program, BSN preferred.
• Current RN license in applicable state(s).
• At least two years of nursing experience, including one in home care; supervisory or case management experience preferred.
• Current CPR certification.

• Strong clinical assessment and care planning skills.
• Excellent communication, organizational, and teamwork abilities.
• Ability to manage multiple clients and adapt to flexible schedules.
