DescriptionRapid rehousing and scattered site housing program for those living with HIV or AIDS.
Duties & Responsibilities- Provide direct case management such as establishing rapport with clients, writing comprehensive treatment plans, and teaching direct skills. Assist with budgeting, medical services, employment, transportation, and other needs.
- Assist clients in completing applications for benefits such as Husky, Social Security, State Supplements, and other entitlements.
- Link clients to job and vocational training opportunities.
- Connect clients to appropriate mental health or substance abuse counseling and DCF services.
- Provide direct assistance with security deposits and time‑limited rental assistance.
- Advocate with landlords for tenant rights and resolve conflicts as needed.
- Demonstrate competency in age‑related or specialty issues and developmental needs for each population served.
- Provide outreach and community education, coordinating access networks and maintaining program awareness in the community.
- Assist clients in understanding tenant rights and responsibilities, basic housing law, and conduct outcome surveys as required.
- Maintain documentation in compliance with agency and program standards.
- Participate in staff meetings, supervision, trainings, and other meetings as assigned.
- Perform additional duties as required.
Education: Bachelor’s Degree required, concentration in Human Services preferred.
Experience: 2 years’ experience working with homeless populations or families dependent on the position.
Licensure/Certification/Registration: Valid driver’s license. CPR/First‑Aid certification desired within 6 months of hire.
Compensation$20.96/hr minimum, commensurate with experience and subject to internal equity policy.
