Patient Care Coordinator - Home Health

Overview

Atlas Medical is looking for a full‑time Patient Care Coordinator (PCC) to work onsite. The role supports patient care by serving as the primary point of contact for referral sources. You will gather essential patient information, handle authorizations, verify insurance, and maintain confidentiality.

• Collect vital patient data and coordinate necessary authorizations.
• Verify insurance coverage and maintain privacy compliance.
• Serve as a central communication hub between patients, families, and referral sources.
• Process referrals and manage referral information accurately.
• Maintain organized records of patient interactions and documentation.

• 1–2 years medical office experience preferred.
• Strong verbal and written communication skills.
• Excellent time‑management and multitasking abilities.
• Valid driver’s license, current insurance, and reliable transportation.
• Positive, compassionate attitude and service‑oriented mindset.
• Clean criminal background check, current CPR certification, and a negative TB test.
