Nurse Practitioner

Job Overview

Mosaic Healthcare is a community based palliative care program. We take care of patients who are not quite hospice eligible or have chosen to pursue aggressive care. The focus of this position will be on providing compassionate care to patients who are suffering from chronic illness and severe disease burden.

• Evaluation and treatment of chronic illnesses (CHF, COPD, HTN, DM, Dementia, Chronic Debility, Malnutrition, Chronic Pain, etc.)
• Understand and manage chronic illnesses
• Manage chronic symptoms (Pain, Nausea, Diarrhea, Constipation, Shortness of Breath, Edema, etc.)
• Valid DEA and prescribe opioids. Comfortable managing and treating chronic pain due to terminal conditions.
• Responsible for having treatment goal planning conversations with patients and families, discussing end of life, life limiting conditions, advanced care planning, code status discussions, etc.
• Comfortable working with physician consultants and teams. Patients may have primary care physicians as well as specialist groups.
• Training will be provided for the right individual.
• Employment will include education from palliative care physicians, other nurse practitioners, and company‑prepared training.
• Malpractice insurance will be provided.
• Expected to see a defined caseload daily. Work settings include hospital, clinics, and occasional home visits.
• Share weekend call with physicians and other nurse practitioners. Round one weekend a month at a hospital facility.

Job Type: Full‑time

Pay: $90,000.00 – $ per year
