Role DescriptionWe provide effective and equitable value‑based primary care to adult Medicare patients. Our nurse practitioners see patients independently, collaborate with physicians, and support care coordination and population health initiatives.
Core Responsibilities- Provision of exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments, maintaining a holistic view of health and accurate documentation.
- Coordinate care with other providers, specialists, testing facilities, and agencies.
- Lead population health initiatives in coordination with the Care Team (e.g., ensuring eligible females receive evidence‑based breast cancer screening every two years).
- Assist the care team with phone triage and outreach.
- Educate patients on their health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
- Perform other duties as assigned.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification in at least one of the following specialties:
Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner. - Active, non‑probationary state Nurse Practitioner license.
- Active DEA license.
- US work authorization.
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understanding of HCC (Hierarchical Condition Categories) documentation, ICD‑10 coding, and Health Risk Assessments (HRAs).
- Passion for teamwork and cross‑functional collaboration.
- Desire to be part of an innovative, empirically‑guided population health model.
- Bilingual proficiency in applicable areas.
40 hours per week, full‑time.
Pay Range$95,738.00 – $ (base hourly rate or annual salary, varying by experience, education, geography, and other factors).
