In-Home Health - Nurse Practitioner or Physician Assistant Bridgeport, CT

About the Role

Signify Health, part of CVS Health®, is looking for a full‑time Clinician (Nurse Practitioner or Physician Assistant) who will deliver in‑home health evaluations, engaging with patients in the comfort of their own homes and bridging gaps in care.

• Bring heart into every visit and build trusted relationships with patients.
• Deliver in‑home evaluations lasting about 45 minutes, including medication and medical history review, physical assessment, and any ordered diagnostic tests (e.g., diabetic eye exams, spirometry).
• Use an iPad and intuitive clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).
• Travel locally and within licensed states as required.
• Maintain a safe and comfortable environment for patients, accommodating pets, secondhand smoke, or allergens.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Evidence of professional experience (NP or PA).

• Previous experience in home health or preventive care settings.

The typical pay range for this role is: $95,738.00 – $. This range represents the base hourly rate or base annual full‑time salary and will be adjusted based on experience, education, geography, and other factors.
