Job Overview

We provide skilled nursing care to people recently discharged from the hospital, needing therapy and those needing long‑term care.

• Extra Shift Differential Pay: $2.00 Sun–Thu & $3.00 Fri–Sat
• Staggered Weekend

  3‑11 p.m. Every Other Weekend; 7–3 p.m. Every Weekend

• Assist with implementation and revision of care plans; monitor health conditions; supplement direct care and respond to emergencies or changes in status
• Observe care assignments, manage coverage needs, evaluate staff performance, educate and lead staff
• Communicate changes in resident conditions to families, providers, and the interdisciplinary team
• Review and investigate incidents; transcribe orders; audit medical charges; maintain record compliance for accuracy
• Explain health status, changes, and expectations for care; guide and direct care skills and interventions as needed

• Licensed to work within the State of Connecticut as an RN Registered Nurse
• Must pass background check, drug screen, professional references check, and physical evaluation
• Demonstrated leadership and clinical competency
