Come join our team! The Greater Waterbury YMCA is currently seeking a CHAP/CHEER Case Manager.

Job Description

This position supports the work of the Y, a leading nonprofit, charitable organization committed to strengthening community through youth development, healthy living and social responsibility. The CHAP/CHEER Case Manager at the Greater Waterbury YMCA provides case management for clients in the CHAP/CHEER program.

Qualifications
  • Bachelor’s Degree in closely related field (Social Work, Secondary Education or Counseling)
  • Experience in teaching and reinforcing life skills and assistance with community linkages for development of natural supports
  • The ability to work professionally with others, as well as autonomously
  • Minimum of two years of experience working with youth.
  • Excellent interpersonal communication and writing skills, well‑organized, the ability to manage your time effectively, and operate business computer applications at an intermediate level (Microsoft Word, Excel, Business Email Systems, etc.)
  • Must have and maintain a clean CT Driver’s License, current state registration and be the policy holder of full auto insurance coverage
Essential Functions
  • Provide 5-40 hours of case management services
  • Must have safe, reliable vehicle. Mileage reimbursement paid monthly.
  • Teach and reinforce all life skills documented on the Life Skills Inventory checklist, including budgeting, housing, education and employment
  • Be knowledgeable of community resources and provide information and assistance with community linkages for development of natural supports
  • Assist clients with finding and enrolling in suitable secondary education and obtaining financial aid
  • Monitor educational status including attendance and grades
  • Assist clients with preparing a resume and job searches
  • Assist new clients and those wishing to move with locating and maintaining housing
  • Maintain knowledge about program guidelines and DCF policies and procedures
  • Ensure required documentation and reports are completed and presented as required
