A not-for-profit healthcare provider in Waterford, CT seeks a Licensed Practical Nurse (LPN) for homecare. The LPN will provide skilled nursing care for patients at home, under the supervision of a Nurse Case Manager. Responsibilities include coordinating care, assisting clients with social needs, documenting care, and communicating effectively with clients of all ages. The ideal candidate will be a graduate of an NLN accredited nursing school, hold a Connecticut Practical Nurse license, and have at least one year of acute care experience.
Homecare experience is strongly preferred.
