Home Health RN – Independent Patient Care

Yale New Haven Health is looking for a Home Care Nurse to manage professional nursing care as per physician orders. This position involves developing care plans, coordinating with clients and families, and ensuring a safe environment for clients. Applicants should have a nursing diploma or an associate's degree, with preference given to those with a bachelor's degree.

Two years of nursing experience is preferred, along with a valid Connecticut RN license. The role also requires excellent clinical and organizational skills, with the ability to work independently in home care settings.
