Registered Dietitian Nutritionist
The Registered Dietitian Nutritionist spends approximately half of their time providing Diabetes Self-Management Education and Support (DSMES), including Medical Nutrition Therapy and individualized diabetes education. The remainder of their time is spent providing clinical Medical Nutrition Therapy for patients with other nutrition-related conditions, interdisciplinary care coordination, and supporting organizational nutrition initiatives. Performs all defined services and other related duties in accordance with the mission of Peoples Community Health Clinic.

Diabetes Care and Education (CDCES)- Provide individual and group Diabetes Self-Management Education and Support (DSMES) consistent with current evidence-based standards.
- Develop and deliver individual and group diabetes education that addresses healthy eating, carbohydrate counting, medication adherence, physical activity, behavior change, glucose monitoring, continuous glucose monitoring (CGM), insulin administration, insulin pump therapy, and other diabetes technologies, as appropriate.
- Interpret glucose data, laboratory values, and nutrition-related clinical indicators to develop and monitor individualized diabetes management plans.
- Collaborate with the interdisciplinary care team to optimize diabetes outcomes and support diabetes-related quality improvement initiatives.
- Conduct comprehensive nutrition assessments utilizing medical, nutritional, behavioral, social, cultural, and environmental information.
- Develop, implement, monitor, and modify individualized nutrition care plans using the Nutrition Care Process and evidence-based practice guidelines.
- Provide Medical Nutrition Therapy for patients with nutrition-related conditions across the lifespan, including maternal, pediatric, adult, geriatric, and oral health-related nutrition needs.
- Document nutrition assessments, interventions, patient education, and follow-up care within the electronic health record.
- Collaborate with providers and members of the interdisciplinary care team to develop coordinated patient care plans, provide nutrition consultation, and support evidence-based nutrition interventions.
- Collaborate with the Community Nutrition Specialist to integrate Food Is Medicine, Produce Prescription, and other nutrition access programs into individualized patient care plans, coordinating referrals as appropriate.
- Provide individualized nutrition counseling utilizing motivational interviewing and evidence-based behavior change strategies.
- Develop and deliver culturally responsive nutrition education for patients, families, and caregivers.
- Participate in quality improvement initiatives, population health activities, and development of nutrition-related clinical protocols and evidence-based practice guidelines.
- Monitor and support achievement of organizational quality measures related to nutrition services, including diabetes, hypertension, obesity, preventive care, and UDS reporting.
- Maintains current and quality written nutrition services materials, policies, and procedures.
- Supports a service-oriented atmosphere in accordance with Peoples Community Health Clinic Mission and Philosophy.
- Works to improve work processes and clinical outcomes including health disparity and quality improvement collaboratives.
- Follows policies and procedures.
- Maintains a safe working environment and practices safe working habits.
- Assists in control of Peoples Community Health Clinic resources.
- Strong analytical, problem-solving, and reasoning skills and ability

- Maintains strong clinical knowledge within RDN scope of practice
