Schedule
Full-Time (1.0 FTE)
Monday – Friday | 8:00 a.m. – 4:30 p.m.
Additional requirements:
Average of 2–3 weekends per year1–2 weeknights per month of on-call coverage
Participation in a holiday on-call rotation
What You'll DoProvide ongoing assessment of the physical, emotional, and mental statuses of patients/families.
Provide direct nursing care under the direction of an RN.Use teaching, assessment, observation, and intervention skills to provide comfort and care maximizing patient/family quality of life.
Assist the RN in monitoring, observing, and reporting reactions to therapy and communicates responses to interventions promptly to primary RN.Qualifications Required Current Iowa state LPN license.
Positive attitude and team player.
Demonstrate good written and verbal communication skills.
Basic computer skills with the ability to learn work-related software and have basic typing skills.
Current Iowa driver’s license, reliable vehicle, and car insurance.
Preferred3-5 years of recent experience as a nurse in a related field.
1 year of experience in hospice or home care nursing.
Experience working in a team environment.
Physical Requirements Ability to move up to 50 pounds with or without reasonable accommodation
Ability to travel throughout the service area and work in patient homes, healthcare facilities and office settings
Ability to alternate between sitting, standing, walking and driving throughout the workday
Pre-employment/post-offer physical, TB, and drug screen required
