The Hospitalist Nurse is a registered nurse who collaborates with the hospital care team and floor staff to coordinate and manage the process of patient care from admission through discharge and hand‑off to the primary care provider.

• Requires bedside marketing to educate patients & their families on the role of the Hospitalist Program, verifies primary care providers, and determines any barriers to discharge & sets expectations for hospital stay.
• Actively participates in morning report/hand‑off and can serve as census coordinator.
• Collaborates with the hospital care team to ensure all labs, test results, consults, orders, etc. are completed in a timely fashion.
• Initiates proactive planning and coordination of the patient’s plan for the day and the stay with the rest of the team.
• Participates in quality improvement initiatives; reviews core measure indicators and ensures team goals are met.
• Assesses with patients & their family members that their needs are met & collaborates to set up follow‑up needs upon discharge.
• Obtains medical records & assists with medication reconciliation.
• Occasionally sets up care‑team meetings with physicians & families about ongoing hospital care, safe discharge plans & disposition.
• Provides continuity for patients during physician hand‑offs.
• Collaborates with physicians and care coordinators regarding care delays.
• Resolves post‑discharge issues needing follow‑up & provides post‑discharge homecoming calls to high‑risk patients.
• Educates patients at the bedside and over the phone; recognizes limits of when to collaborate with a physician regarding patient care either in the hospital or during a homecoming call.

• Graduate of an accredited Registered Nurse program.
• Current license to practice nursing in the State of Iowa or Illinois.
• Meets educational/competency requirements per policy.
• Ability to use the computer effectively to navigate several electronic programs related to patient care.
• Mandated Reporter certification.
• CPR certification.
• Strong interpersonal skills.
• Ability to work as a team member.
• Ability to understand and apply guidelines, policies and procedures.
• 3‑5 years of nursing experience preferred.
• Bachelor’s education in a health‑related field preferred.
• Strong verbal and written communication skills.
