Host Home Supervisor; Contract Monitor - Eastern Iowa Region

Our Services for People with Disabilities is expanding its team and is searching for a dedicated, energetic full‑time supervisor to work as a Contract Monitor for our Host Home Services in the Eastern Iowa region. The position is based in the Cedar Rapids, Cedar Valley or Dubuque area, offers some work‑from‑home flexibility and a travel component.

Covered counties include Allamakee, Benton, Black Hawk, Bremer, Buchanan, Butler, Cedar, Chickasaw, Clayton, Clinton, Delaware, Dubuque, Fayette, Floyd, Howard, Jackson, Johnson, Jones, Linn, Louisa, Muscatine, Scott, Winneshiek. Travel outside these counties may be required to cover service needs and meetings.

• Act as the main contact for interdisciplinary team members and for individuals served in the host home program.
• Work directly with assigned host homes to provide support, assistance, and instruction that facilitate personal growth and independence.
• Facilitate and participate in interviews and evaluations related to the contracting process for independent providers.
• Oversee contractual compliance and ensure that assigned independent providers deliver quality care and meet contractual requirements.
• Work Monday–Friday business hours, with occasional flexibility to work outside regular hours, including some evenings or weekends, as needed; participate in an on‑call rotation.

• Minimum: HS Diploma/GED and five years of related experience, OR Associate’s Degree and three years of related experience, OR Bachelor’s Degree and one year of related experience.
• Preferred:
  Bachelor’s Degree in a human services related field and one year of related experience; supervisory or leadership experience is also preferred.
• Valid driver’s license, auto insurance, good driving record.
• Ability to pass an extensive background check including the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.
