Job Responsibilities
- Provides personal care and housekeeping assistance to our clients in their homes; follows the established plan of care under the direction of the client care coordinator.
- Provides personal hygiene and grooming care to our clients
- Provides meals for clients' nutritional needs
- Reports changes in clients' condition to client care coordinator or supervisor
- Performs routine housekeeping and home management duties
- Records each client's care on care plan and submits care plan in a timely manner
- Responds sensitively to the multiple, complex needs of a terminal client which includes providing end of life care.
- Assists with routine therapy exercises
- Education:
High School diploma or equivalent, preferred - Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services required
- Experience:
Six months experience as Home Care Aide/CNA in a home care agency, preferred
