Nurse Practitioner

Part‑time Clinician (Nurse Practitioner or Physician Assistant)

Signify Health is seeking a Part‑time Clinician to conduct In‑Home Health Evaluations. The role focuses on helping patients in the comfort of their own homes, providing medication and medical history review, physical assessment, and, when ordered by a health plan, additional diagnostic tests such as diabetic eye exams, spirometry, bone density screening, DEE, etc. Clinicians use an iPad and an intuitive clinical workflow.

The clinician is not required to prescribe medication, order lab tests, or alter a patient’s current treatment plan.

• Conduct 45‑minute evaluations that include medication and medical history review, physical assessment, and any ordered diagnostic tests.
• Engage patients one‑on‑one to answer health questions and provide education.
• Perform diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Be flexible to travel locally and within licensed states, and conduct virtual visits as needed.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification required.
• Multi‑state licenses preferred.
• Ability to cover primary locations including Hartford, CT.
• Applicable to Nurse Practitioners and Physician Assistants.

Pay range: $46.03 – $99.14 per hour, depending on experience, education, and geography.
