Job Title

Part Time Clinician (Nurse Practitioner or Physician Assistant) – at Signify Health, a part of CVS Health.

Hartford, CT; local travel and within licensed states; occasional virtual visits.

In-Home Health Evaluations, meeting patients at home, covering medication and medical history review, physical evaluation, and diagnostic tests as ordered by the health plan. Use an iPad and an intuitive clinical workflow. Not required to prescribe medicine, order lab tests or alter current treatment regimens.

• Fulfill the requirements of the specified clinician role.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services such as spirometry, DEE, bone density screening, etc.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state license(s) (preferred).
• Ability to cover the primary location:
  Hartford, CT.
• Time commitment: 29.5 hours per week (part time).

• Pay range: $46.03 – $99.14 per hour.
