PPV Primary Care Nurse; RN, Registered Nurse, Visiting Nurse - HomeCare

PPV visit position. Minimum 10 visits per month. Must complete the weekday orientation process for training. Length may vary based on prior experience. Covers the greater Waterbury area.

• Provide skilled nursing care to clients in home and community-based settings, ensuring outcome-based, high-quality, cost-effective care.
• Maintain client records using the agency’s standard documentation system.
• Conduct follow-up visits to assess disease progress and evaluate treatment response.
• Offer support to patients and families, applying teaching principles to promote and restore health and prevent disease and disability.
• Coordinate care with providers and consult with physicians regarding client needs and care plans.
• Communicate and confer with the Nurse Care Coordinator and all staff involved in the patient’s care.
• Direct LPNs and Home Care Aides in carrying out the established plan of care, supervising and guiding them as needed.
• Refer clients and families to community resources when required.
• Attend and participate in in-service education programs.
• Teach patients and reinforce client/caregiver education.
• Collaborate and coordinate all client care services.

• Associate’s Degree or diploma program in nursing.
• One year of nursing experience (or one year in a specialty).
• Registered Nurse license to practice in the state of CT.

• Graduate of an NLN approved BSN program.
• Medical/surgery or home care experience.
