Shift Detail: Mon - Fri 8:00‑4:30 covering Ansonia/Deby and Naugatuck Valley. Coverage required: 1 weekend per month, 1 major holiday, and 2 minor holidays per year.

Role OverviewUnder the direction of the RN, this role will provide skilled nursing care to clients in home and community‑based settings, ensuring the delivery of outcome‑based, high‑quality, cost‑effective care. This role will collaborate with all team members to achieve exceptional outcomes.
Responsibilities- Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
- Provide support to patients/families utilizing teaching principles to promote and restore health and prevent disease and disability.
- Record daily progress in EMR.
- Report to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
- Communicate and conference with the primary nurse and other disciplines.
- Consult with physician regarding client needs and plan of care.
- Assist in evaluation of care given by paraprofessional staff.
- Participate on agency committees and special projects.
- High School diploma or GED.
- Successful completion of a CT state‑approved LPN certificate program.
- Computer skills, including Outlook, Microsoft Word, and Excel.
- Current and valid LPN license.
- Valid CT driver’s license with a good driving history.
