Location: 680 Main Street, Watertown (10326)
Shift: Float position for Watertown office coverage area. Mon–Friday 8:00–4:30. Coverage one weekend per month, 1 major and 1 minor holiday per year.
Visiting Registered Nurse – Primary Care Nurse providing personalized care in patients’ homes.
Job Description- Perform comprehensive assessments to establish an individualized care plan.
- Facilitate communication between the patient and the interdisciplinary team.
- Develop and maintain a therapeutic relationship by providing education and promoting self‑advocacy and independence.
- Utilize state‑of‑the‑art technology to document skilled services provided.
- Registered Nurse with valid CT RN license.
- One year of RN experience; home‑care experience preferred.
- Valid driver’s license and reliable transportation required.
- Computer proficiency required.
