Beth Israel Lahey Health at Home is seeking a dedicated Hospice LPN Team Resource in Watertown, MA. You will perform hospice nursing visits, contribute to assessment and care planning, and collaborate with the RN Case Manager to ensure high-quality patient care in a home setting.
You will hold an active MA LPN license, maintain BLS, and bring 2+ years of LPN experience in acute, rehab or home care. Hospice experience is preferred; weekend/holiday rotations are included.
