Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer flexible work options and outside of the box opportunities that focus on your needs and interests.
Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.
Coverage needs primarily in and around the Cambridge/Watertown, MA area and surrounding towns.
- Tuesday-Friday
- 32 hours/week
- No weekends or holidays required
*** Requires minimum 1 year recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.****
