Licensed Practical Nurse

Carthage Center for Rehabilitation and Nursing in Carthage, NY is seeking a compassionate Licensed Practical Nurse to provide high-quality care to our short-term rehab and long-term care residents. The LPN will administer medications and treatments, monitor resident conditions, document accurately, and collaborate with RNs, therapists, and physicians to support individualized care plans. You will educate residents and families, support ADLs, and help maintain a safe environment.

Responsibilities

• Provide direct nursing care to short-term rehab and long-term care residents
• Administer medications and treatments per provider orders and facility policy
• Monitor, document, and report changes in resident conditions
• Support residents with activities of daily living and mobility
• Collaborate with RNs, therapists, and physicians on care plans
• Maintain accurate, timely electronic and written documentation
• Educate residents and families on care, treatments, and health needs
• Promote a safe, clean, and dignified environment for residents
• Follow infection control, safety, and regulatory guidelines
• Contribute to a collaborative, respectful, resident-focused team culture
