Description of Position:

This position serves as a health resource for individuals and works closely with agency staff to promote health, independence, safety, and overall well-being.

Job Knowledge and Comprehension:

Provide nursing support, first aid, CPR, and medical follow-up as needed.

Assist with medication administration and coordinate medication coverage when absent.

Schedule and accompany individuals to medical, dental, and other healthcare appointments.

Communicate with healthcare providers and assist individuals during appointments.

Complete health assessments, including vital signs, height/weight, and screenings.

Maintain accurate medical documentation and records.

Follow applicable nursing standards and agency requirements.

Support individuals' physical, emotional, intellectual, and spiritual health needs.

Perform other duties consistent with the position.

Physical Requirements:

This position requires frequent driving, bending, stooping, climbing stairs, pushing/pulling, and assisting with transfers. Lifting over 50 lbs. may occasionally be required during medical emergencies or behavior support situations.

Current South Dakota LPN license.

Valid driver’s license and safe driving record.

Successful completion of a criminal background check.

Successful completion of a pre-employment drug screening.

The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply.
