Work Location/Program Overview:
Ascentria’s mental health case management services provides support to individuals living in the community. Case Managers assist adults with mental health challenges to live independently. The work is done out in the local community and case managers can substantially impact the lives of the individuals they work with.
Position OverviewThe Adult Mental Health Case Manager plays a crucial role in identifying, assessing, and evaluating services for individuals, ensuring their health and safety to promote independent living. Their focus is on recovery strategies, community support, and advocating for client rights while encouraging self-advocacy. A MHRT-C or MHRT-C Provisional certification is necessary for this position. The work schedule is Monday to Friday from 8:00 AM to 4:00 PM.
Whatwe’re looking for
- A High School Diploma or GED is mandatory; a related associate's degree is preferred.
- A minimum of 6 months of relevant experience is desired.
- A valid state-issued driver's license and proof of current insurance are required.
- Must possess an MHRT-C or MHRT-C Provisional certification and maintain it with 18 approved CEUs every two years.
