Supervisor, Behavioral Health Homecare & Hospice

Schedule:
40 hours per week
- What Youll Do
- Provide clinical supervision, direction, and mentorship to behavioral health and social work staff.
- Support psychosocial assessments, treatment planning, crisis intervention, and bereavement services.
- Review documentation and ensure adherence to best practices and ethical standards.
- Serve as a resource for complex cases, including family dynamics and end-of-life care planning.
- Perform staff evaluations and assist with performance management and disciplinary actions.
- Care Coordination & Discharge Planning
- Lead and participate in interdisciplinary team (IDT) meetings to integrate behavioral health and social work into patient care plans.
- Coordinate care across providers, ensuring seamless transitions between services and settings.
- Support discharge planning and continuity of care for patients and families.
- Facilitate access to community resources and advance care planning.
- Team Leadership & Operational Oversight
- Hire, train, mentor, and evaluate social work and behavioral health staff.
- Determine, coordinate, and supervise daily staffing assignments and coverage levels.
- Facilitate staff development programs and ongoing training initiatives.
- Monitor team productivity, patient satisfaction, and quality metrics, implementing improvements as needed.
- Ensure compliance with departmental policies, Medicare/Medicaid regulations, and state requirements.
- Lead resolution of complex operational and patient care issues.
- Licensure
Required:
Maine state licensure as Licensed Clinical Social Worker (LCSW). - Education: Masters Degree in Social Work (MSW) from a CSWE-accredited program strongly required.
- Experience: 3–5+ years in hospice, home health, or palliative care; 1–2+ years of leadership or supervisory experience.
- Knowledge &
Skills:
Strong knowledge of hospice philosophy, Medicare/Medicaid regulations, and psychosocial care; excellent communication, leadership, and conflict resolution skills; ability to manage multiple priorities in a fast-paced, patient-centered environment.
- Lead a mission-driven behavioral health and social work team across both homecare and hospice.
- Make a meaningful impact on patients and families during critical life moments.
- Blend clinical expertise with leadership and operational influence.
- Work within a collaborative, interdisciplinary care model.
