Job Summary:

Works as part of a multidisciplinary Assertive Community Treatment (ACT) team providing comprehensive, community-based services to adults living with severe and persistent mental illness. Supports clients in achieving recovery goals, maintaining housing stability, reducing hospitalizations, and improving overall quality of life. The majority of work is performed in the community, meeting clients where they live, work, and engage in daily activities to provide individualized support, care coordination, advocacy, and resource connection.

About the ACT Team:

Assertive Community Treatment (ACT) was originally developed as a "hospital without walls," providing intensive, community-based support to individuals at high risk for hospitalization, homelessness, and involvement with the legal system. Our team delivers services where clients choose to live, work, and spend their time, helping them build independence and achieve their recovery goals. As part of our multidisciplinary team, you'll work alongside dedicated and passionate professionals committed to providing person-centered, recovery-focused care.

What You'll Do:

Provide community-based case management services to adults with severe and persistent mental illness.

Meet clients in their homes and throughout the community to assess needs, provide support, and promote recovery and independence.

Collaborate with psychiatrists, nurses, social workers, counselors, peer support specialists, and other team members to develop and implement individualized treatment plans.

Assist clients with accessing healthcare, housing, employment, benefits, transportation, and other community resources.

Monitor client progress, identify barriers to treatment and recovery, and advocate for services that support client goals.

Participate in crisis intervention, hospitalization prevention, discharge planning, and transitions of care as needed.

Maintain accurate and timely documentation in accordance with organizational, state, and federal requirements.

Consult regularly with the multidisciplinary team to evaluate client status, treatment effectiveness, and ongoing service needs.

Respond to questions and concerns from clients, family members, community partners, and other stakeholders regarding care and services.

Operate a motor vehicle to conduct Maine General Health business and provide services throughout the community.

Required Qualifications MHRT/C (Community) certification through the Maine Department of Health and Human Services (DHHS) required.

Valid driver's license and reliable transportation required.

Experience working with individuals living with severe and persistent mental illness preferred.

Compensation Information:$22.65 - $39.64

The hourly pay range shown represents the base pay range for this position. Individual pay is determined based on factors such as experience, qualifications, skills, and market considerations. In addition to base pay, eligible team members may earn additional compensation through shift, weekend, holiday, on-call, and other applicable differentials.
