Hours:

35 per week. Shift: 5x8 Days (8:00 a.m. to 4:30 p.m.). This role provides direct PT services to residents in a SNF, focusing on rehabilitation and functional mobility within a geriatric care environment.

• Conduct initial evaluations and ongoing reassessments to guide individualized treatment plans
• Deliver PT interventions including therapeutic exercises, gait and balance training, ROM advancement, and functional mobility activities
• Monitor progress, adjust treatment plans, and document outcomes clearly in resident medical records
• Educate residents and families on home exercise programs, safety strategies, and fall prevention
• Coordinate care with nursing, therapy colleagues, and the interdisciplinary team to optimize rehabilitation goals
• Ensure patient safety, apply infection control practices, and maintain compliant documentation and regulatory standards

• Active Physical Therapist license in the United States (eligible for licensure in Maine as applicable)
• Ability to work the stated shift schedule in a Skilled Nursing Facility environment
• Strong assessment, documentation, and communication skills
