Responsibilities
  • Provide hands‑on nursing care to patients as an LPN, per care plans.
  • Document patient status and update charts with accuracy and timeliness.
  • Administer medications and treatments under physician orders and policy.
  • Collaborate with healthcare team to coordinate patient care and outcomes.
  • Monitor patient responses and elevate concerns per protocol.
  • Adhere to regulatory and safety standards, including infection control.
  • Proactively identify opportunities for professional development and skill advancement.
  • Meet performance targets and contribute to pipeline‑driven goals for the area.
  • Educate patients and families on care plans, treatments, and follow‑up steps.
  • Maintain accurate mileage, time, and activity logs for performance visibility.
  • Participate in on‑site team meetings to review progress and share insights.
Experience/Qualifications
  • Active LPN license in the relevant state with current CPR certification.
  • Experience providing hands‑on nursing care per care plans and procedures.
  • Proficiency documenting patient status and updating electronic health records accurately.
  • Ability to administer medications and treatments under physician orders and policy.
  • Strong collaboration skills with healthcare team to coordinate patient care.
  • Observation skills to monitor patient responses and elevate per protocol.
  • Commitment to regulatory, safety, and infection control standards.
  • Demonstrated drive for continuous professional development and skill advancement.
  • Record‑keeping excellence for mileage, time, and activity logs for performance visibility.
  • Willingness to participate in on‑site team meetings and share insights.
