The Clinical Team Coordinator acts as central point of contact for all patients and clinical staff to sustain efficient team operations and facilitate top of license work by clinical staff. The incumbent coordinates meetings and employee activities as directed and acts as a resource in facilitating the supply management process. The Clinical Team Coordinator is responsible for the coordination of initial patient care assignments, assisting with ongoing scheduling, and addressing patient concerns/needs for the team.
The incumbent provides general clerical support as directed. Applies clinical knowledge to triage patient symptoms and provides education telephonically. The incumbent utilizes critical thinking, clinical skills, and problem‑solving skills to promote continuous quality improvement in the area of supporting the patient care experience.
- Triages phone calls and processes physician orders as appropriate for the level of current licensure/certification.
- Consistently uses electronic health record (EHR) and electronic resources to communicate with internal and external customers.
- Assigns patient visits and updates staff schedules in the EHR as necessary to support patient care needs.
- Obtains referrals from central intake and assigns clinical team member to patient based on geographical region and clinician availability, utilizing clinical knowledge to support clinical team needs.
- Collaborates with department leadership to prioritize visits that are unassigned due to staff absences and fluctuations in patient volumes, and subsequently collaborates with clinical staff regarding work assignments, when changes are required.
- Generates and distributes team reports as directed.

- Ability to multi‑task in a fast‑paced environment, utilizing electronic resources.
- If assigned to a Hospice clinical team, assists in obtaining authorized DME/supplies for Hospice patients as requested, verifies de‑authorization of Hospice medical supplies/equipment and pharmaceuticals completed upon patient discharge.
- Valid driver's license with reliable transportation and agency required auto liability insurance. Must be able to drive within assigned region as directed. Statewide travel may be required occasionally.
- BLS for Healthcare Providers certification is required to be obtained prior to independent visits.
- 2+ years of relative work experience required.
