Job Summary:
Provide comprehensive nursing care to patients in a Long-Term Care or Skilled Nursing Facility setting. Ensure high-quality, patient-centered care while complying with facility policies and healthcare regulations.
Responsibilities:
- Deliver direct patient care by monitoring vital signs, administering medications, and managing treatments.
- Assess patient conditions and develop individualized care plans in collaboration with the healthcare team.
- Document patient progress accurately in electronic health records.
- Educate patients and families on health management and discharge instructions.
- Adhere to infection control protocols, including COVID-19 safety procedures.
- Maintain compliance with state and federal regulations concerning healthcare practices.
Qualifications:
- Valid and current Registered Nurse license.
- Minimum of one year experience in LTC/SNF settings preferred.
- Ability to comply with facility and safety policies, including COVID-19 vaccination requirements.
- Strong assessment and communication skills.
- Proficient in Electronic Health Record (EHR) systems.
- Must present a valid government-issued be able to wear a photo badge at all times.
