Position SummaryAs a Nurse Practitioner (NP) with Impact Health PC in Waterville, ME, you will provide comprehensive care to patients within skilled nursing facilities. You will assess and manage acute and chronic conditions, develop treatment plans, and collaborate with physicians, facility staff, specialists, and interdisciplinary care teams.
Key Responsibilities- Assess, diagnose, and manage acute and chronic conditions.
- Develop evidence-based treatment plans.
- Complete timely clinical documentation and place appropriate orders.
- Collaborate with physicians, nursing staff, specialists, and care teams.
- Educate patients and families regarding conditions and treatment plans.
- Maintain compliance with facility, state, and federal regulations.

Qualifications- Active Nurse Practitioner (NP) license in Maine.
- SNF, hospitalist, emergency department, or ICU experience preferred.
