Maine General is looking for a dedicated Home Health Case Manager RN to join our Augusta team. In this role, you'll provide individualized patient care in the comfort of their homes, fostering strong patient relationships while promoting health and independence.

What You'll Do

Collaborate with nursing staff and care team regularly to assess patient status and care decisions like admission, length of stay, transfer, and discharge.

Deliver direct treatment, education, and health condition management, advising providers on additional services and community referrals.

Develop, implement, and assess patient care plans based on assessments to improve outcomes. Monitor, record, and communicate patient condition as needed.

What You'll Need
  • Active Maine RN license (or the ability to obtain one promptly).
  • At least 1 year of acute care, med/surg, or equivalent nursing experience (home care, ED, ICU, or CCU experience preferred).
  • Valid driver’s license & acceptable driving record.
