DescriptionMust be a C.N.A. to apply. St. Mary's Home Care & Hospice is seeking a full time Certified Home Health aide/C.N.A.
$2,000 Sign On bonus offered (

).
Our hospice/home health aides provide paraprofessional services to patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
What You Will Do- Follow care plan developed by RN.
- Report client changes in condition to RN, as described on care plan.
- Assist clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
- Assist clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
- Measure and record client’s oral, rectal and axillary temperatures, pulse, respiration and blood pressure as ordered.
- Perform range of motion and other simple procedures as an extension of therapy services as assigned.
- Assist clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind.
- High School diploma or GED required.
- Completion of CNA 75 hr course.
- Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
- Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
- Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references).
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
- Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on‑going basis.
- Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
- Must have current Driver’s license and reliable transportation to and from work site.
