Hospice weekend RN

Position Overview

Full-time Home Care Baylor Registered Nurse with St. Mary Home Care & Hospice (Trinity Health at Home) in Athens, GA.

Shift: Three 12-hour shifts (paid 40 hours). Works Saturdays & Sundays with a Friday or Monday included.

Sign-on Bonus: $10,000 (not applicable to internal employees, former employees who resigned in the last year, or applicants sourced from outside firms).

• Provide primary nursing care to home-based care patients as ordered by the physician, in accordance with organizational policies and procedures.
• Conduct initial assessment and develop care plan in collaboration with the Population Health Case Manager.
• Provide functional support/supervision to Home Healthcare Aides and LPNs as needed.
• Deliver therapeutic intervention, overall care management, and achieve top decile client outcomes and patient satisfaction.

• Graduate of an approved nursing education program.
• Registered Nurse licensure in the state of PA.
• One (1) year experience as a professional care nurse.
• Current driver's license and reliable transportation.
