This position works three 12 hour shifts/ paid 40 hours. Working Saturday & Sunday with a Friday or Monday included ***$15K Sign On Bonus***
St. Mary Home Care & Hospice, a member of Trinity Health at Home, has an immediate need for a Full-time Baylor Registered Nurse for the greater Athens area & surrounding region.
Provide primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization's policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support/supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.

Minimum qualifications
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the state of Georgia
- One (1) year experience as a professional care nurse
- Must have current Driver's license and reliable transportation
