Hospice RN Case Manager
St. Mary's Home Care & Hospice is seeking a full time Hospice RN Case Manager. ***$10K Sign on bonus offered****. Signing bonus is not applicable to internal employees, former employees who have resigned in the last year or applicants sourced from outside firms.
Hospice RN Case Manager position summary:
Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Position is full time primarily Monday-Friday. No on call or weekends.

Minimum qualifications:
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Georgia
- 2 years experience as a professional nurse in acute care, home care or ER experience required
- Home care experience preferred but not required
- EPIC experience preferred
