Flexible Territory Options – Choose One We are hiring for two territory options:

Territory 1:
Athens, Hall, Banks, and Habersham Counties Territory 2:
Clarke County Candidates will be considered for either territory based on their geographic area and patient needs.

What You’ll DoAs a Hospice RN Case Manager, you will play a key role in ensuring patients receive exceptional, individualized care throughout their hospice journey.

Provide compassionate, skilled nursing care to hospice patients in their homes and other care settings

Complete comprehensive assessments and develop individualized plans of care Monitor patient conditions and manage symptoms to promote comfort and quality of life Collaborate closely with physicians, hospice clinicians, caregivers, and families

Educate and support patients and families throughout the hospice journey

Serve as a consistent clinical resource and advocate for your patients

Participate in interdisciplinary team meetings and coordinate patient care Maintain accurate and timely clinical documentation

What We’re Looking For Current RN license in the state of Georgia Hospice, home health, palliative care, or related experience preferred

Strong clinical assessment and communication skills

Ability to work independently while collaborating with an interdisciplinary team Compassionate, patient-centered approach

Valid driver’s license, reliable transportation, and ability to travel within the assigned territory

On An Average Day You Will:(includes, but not limited to)
Make a true difference—one patient, one family, one compassionate moment at a time.

Develop and implement individualized care plans based on current and prior medical history

Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding

Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey

Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure

Administer physician-ordered medications and treatments with skill and empathy

Educate patients and their families on care techniques and symptom management

Track and communicate changes in patient condition, ensuring timely and effective responses

Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided

Collaborate with interdisciplinary teams to ensure comprehensive care delivery
