Santa Cruz County — Watsonville

• Partner with the Case Manager to provide skilled services assisting patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive, and fluency disorders that affect communication.
• Serve as an educator, patient advocate, and member of the interdisciplinary care team.
• Maintain accurate and up‑to‑date client records in electronic charting systems.
• Participate in care team meetings and contribute to treatment plans.

• Current unrestricted California Speech Therapist License.
• Current BLS/CPR Certification.
• Valid California Driver's License.
• Knowledge of individual development, human behavior, and cultural influences on healthcare.
• Understanding of state and federal home‑care regulatory guidelines (preferred).
• Experience in home health care (highly preferred).
• Experience with high‑acuity patients (a plus).
• Experience working with an interdisciplinary team (highly desired).
• Case Management fundamentals (highly desired).
• Self‑directed, empathetic, critical thinker, outstanding interpersonal and leadership skills, and organized.
• Successful completion of a computer literacy and competency assessment prior to interview.
