• Valid nursing license (RN or equivalent) in the relevant state or region
• Proven experience in care planning, case management, or a related nursing role

• Excellent communication and interpersonal skills to collaborate effectively with patients, families, and healthcare teams
• Ability to assess patient needs and develop tailored care strategies
• Organizational skills with attention to detail and ability to manage multiple cases simultaneously
