Position: Counselor II - Residential Clustered Apartment Program
The Counselor II is a valuable member of our behavioral health Residential Support Team who provides comprehensive services and support to chronically mentally ill adults in their homes, the community, and the office for clients residing in the Clustered Apartment Program.

Key Responsibilities

* Provides direct services to clients, including organizing and facilitating individual and group treatment and activities, supervision of the milieu, case management, and outreach.

* Conducts assessments and develops treatment plans with clients.  

* Provides individual and group counseling, skill building, and case management, as well as crisis intervention when needed.  

* Provides education and direction to clients, family members, and/or significant others.

* Works with clients to secure and maintain entitlements, housing, and other needed services.

* Monitors clients to ensure that they are caring adequately for themselves and for the facility and furnishings.

* Demonstrates/teaches ADLs when required.

* Organizes and participates in treatment-related clinical conferences and staffing on assigned clients.  Reports client progress, behavior problems, or need for medication evaluation to program supervisors/physicians/psychiatrists regularly.

Schedule:

37.5 hours a week

* Tuesday to Saturday (8:00 am-4:00 pm
* $5.00 shift differential for working hours deemed eligible for shift and weekend premium

* Bachelor's degree in a related field, or

* Bachelor's in any field and two (2) years' experience, or

* High School diploma or GED and five (5) years' experience

* Valid driver's license and vehicle insurance

* Requires use of personal vehicle when a LCHD fleet vehicle is not available for case management activities, client appointments, crisis response, community outreach or other job-related activities.
