Position Title:

HEALTH SERVICES SUPERVISOR

Accountable to:
Director of Nursing

Primary Objective

Provides support to the Director of Nursing in planning, organizing, directing, and evaluating nursing services throughout the facility. The Health Services Supervisor offers clinical leadership and oversight to nursing staff, promotes high-quality resident care, and ensures adherence to all relevant federal, state, and facility regulations. This role upholds the facility’s mission to deliver safe, person-centered care to individuals living with serious mental illness.

The Health Services Supervisor also contributes to staff education, performance management, quality-assurance initiatives, infection-prevention practices, incident review processes, and survey readiness. The position plays a key role in fostering a therapeutic, recovery-oriented environment for residents.
Flexibility is essential, as the supervisor will assist with direct care on the floor when needed.

Essential Duties and Responsibilities
  • Supports the Mission, Values and Vision of Bayside Care Center and the facility.
  • Is knowledgeable of consumer rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all consumers in a safe, secure environment.
  • Supports, cooperates with, and implements specific procedures and programs for:
  • Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
  • Confidentiality of all data, including consumer, employee and operations data.
  • Quality Assurance and compliance with all regulatory requirements.
  • Supports and participates in common teamwork:
  • Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment.
  • Uses tactful, appropriate communications in sensitive and emotional situations.
  • Follows up as appropriate with supervisor, co-workers or consumers regarding reported complaints, problems and concerns.
  • Promotes positive public relations with consumers, family members and guests.
  • Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned.
Requirements Primary Duties and Responsibilities
  • Assist in overseeing daily nursing department operations.
  • Participate in a designated on-call rotation with the Director of Nursing to provide after-hours clinical support and supervisory coverage.
  • Maintain adequate staffing across all shifts and directly filling floor nurse roles when necessary.
  • Make daily rounds to ensure nursing personnel perform required duties and to ensure appropriate protocols are being followed.
  • Assist in developing a written nursing plan of care for each consumer and actively participate in consumer care plan conferences.
  • Supervise, mentor, and evaluate nursing personnel.
  • Ensure compliance with Illinois Department of Public Health (IDPH), CMS, and facility policies.
  • Monitor resident care, assessments, treatment plans, and documentation.
  • Participate in quality assurance and performance improvement initiatives.
  • Assist with investigations, incident reporting, abuse prevention, and risk management activities.
  • Coordinate staff training and competency assessments.
  • Support infection prevention and control programs.
  • Assist with survey preparation and regulatory compliance activities.
  • Manage the nursing department in the DON’s absence.
Health Services Supervisor Qualifications, Skills, and Abilities
  • Current, active Licensed Practical Nurse (LPN) or Registered Nurse(RN) in good standing.
  • Minimum of two to three years of nursing experience in long-term care, behavioral health, psychiatric services, skilled nursing, or a related healthcare setting.
  • Previous supervisory, charge nurse, or nursing leadership experience required.
  • Knowledge of Illinois Department of Public Health (IDPH), Centers for Medicare & Medicaid Services (CMS), and other applicable federal and state regulations.
