*** Fairfield Memorial Hospital is seeking a compassionate, skilled Registered Nurse to join our Skilled Care Unit. RNs play a vital role in delivering safe, high‐quality, resident‐centered care by assessing needs, developing individualized care plans, and ensuring continuity of care across shifts. This role provides direct nursing care while supervising LPNs, CNAs, and supportive staff to maintain an exceptional care experience consistent with FMH’s mission and standards.

The RN serves as a resident advocate, promotes clinical excellence, and supports residents and families through education, communication, and collaborative care planning.
** KEY RESPONSIBILITIES
*** Conduct comprehensive assessments on admission, ongoing per protocol, and when conditions change; document findings accurately.
* Identify nursing needs, set goals, and implement appropriate nursing interventions based on interdisciplinary care plans.
* Document changes in condition, care provided, and resident responses in the medical record.
* Notify physicians of changes in condition, labs, and diagnostic results; follow through until appropriate actions are taken.
* Review, note, and carry out physician orders promptly and accurately.
* Provide thorough handoff reports and ensure continuity of care across shifts.
* Assign duties to CNAs and LPNs, supervise care delivery, and communicate updates as resident needs evolve.
* Administer medications and treatments safely using the five rights; document per policy.
* Protect resident rights, support dignity, and involve residents and families in care planning.
* Educate residents and families on care guidelines, treatment plans, and discharge needs.
* Remain calm and effective in emergencies, providing leadership and reassurance.
* Follow and supervise infection control procedures to maintain a safe environment.
** COMPENSATION:** $40.00 per hour + Night Shift Differential (Weekday Night: $3.00 per hour; Weekend: $4.00 per hour)

** EDUCATION/QUALIFICATIONS
**
* Required:

Current Illinois Registered Nurse (RN) licensure;
Basic Life Support (BLS) certification.
** EXPERIENCE
* ** Preferred:
Experience in long‐term care or skilled nursing;

Experience with electronic documentation systems.
** SKILLS
* ** Strong interpersonal and communication skills
* Ability to read and communicate effectively in English
* Basic computer knowledge
* Ability to follow written and verbal instructions
