Case Manager, Registered Nurse Field; Bureau County, IL

• Pay: $66,575 - $142,576/year
• Location: Bureau County, IL
• Residency: Must reside in the state of Illinois
• Travel: Willing and able to travel up to 75% of the time

• Develop a proactive plan of care to enhance member outcomes.
• Conduct evaluations of member needs using clinical tools and data review.
• Apply clinical judgment to reduce risk factors and address complex health indicators.
• Conduct assessments considering various information sources, including claims.
• Use a holistic approach to determine the need for referrals to clinical resources.
• Collaborate with supervisors and stakeholders to overcome barriers in care.
• Present cases at interdisciplinary case conferences.
• Utilize motivational interviewing skills to engage members effectively.
• Interact with members/clients telephonically or in person.
• Meet with members/clients in their homes or worksites for ongoing case management.

• Education:
  • Associate's degree required
• Experience:
  • Minimum 3-5 years clinical practical experience
  • Preferred: 2-3 years in Care Management, discharge planning, or home health care coordination
