Boys & Girls Club of Lake County is seeking a compassionate, organized professional to connect youth and families with behavioral health services, healthcare, housing resources, food assistance, and other community supports.

Working closely with families, Club staff, and community partners, the Family Resource & Behavioral Health Coordinator helps youth successfully access services, strengthens community partnerships, tracks outcomes, and improves coordination across our network of providers.

This is a non-clinical position and does not provide counseling, therapy, or clinical treatment.

What You'll DoFamily Resource Navigation
  • Conduct family needs assessments and connect youth and families with behavioral health, healthcare, housing, food security, and other community resources.
  • Coordinate referrals to approved service providers, facilitate warm hand-offs, and follow up with families to promote successful access to services.
  • Assist families with navigating community resources and public benefits, including programs such as SNAP, Medicaid, and All Kids.
  • Monitor referral outcomes, identify barriers to accessing services, and help families overcome challenges to receiving support.
  • Coordinate community engagement activities that connect youth and families with wellness, behavioral health, and other supportive services.
Community Partnerships
  • Build and maintain strong working relationships with behavioral health providers, healthcare organizations, schools, and community agencies throughout Lake County.
  • Coordinate partner-delivered workshops, presentations, and family engagement opportunities.
  • Maintain Memorandums of Understanding (MOUSs) and support the continued development of referral partnerships.
  • Participate in community coalitions and collaborative initiatives that strengthen services for youth and families.
Staff Collaboration & Support
  • Serve as a resource for Club staff regarding community referrals and available family supports.
  • Assist staff in identifying youth and families who may benefit from outside services.
  • Coordinate partner-led trainings on trauma-informed practices, behavioral health awareness, and community resources.
  • Support annual staff refreshers related to mandated reporting, de-escalation basics, and referral procedures.
Documentation & Program Support
  • Maintain accurate referral, service, and outcome documentation within BGCLC's case management system.
  • Track key performance indicators and referral outcomes to support continuous improvement.
  • Assist with collecting documentation and data needed for grant reporting and program evaluation.
  • Ensure documentation is accurate, timely, confidential, and compliant with organizational and funder requirements.
Safety & Crisis Response
  • Follow established procedures when youth or families disclose concerns involving abuse, neglect, suicidal ideation, self-harm, threats of harm, and other crisis situations.
  • Immediately notify supervisors and facilitate warm hand-offs to designated crisis or clinical partners in accordance with BGCLC procedures.
  • Fulfill mandated reporter responsibilities and maintain appropriate documentation.
  • Maintain professional boundaries consistent with the non-clinical scope of the position.
General Responsibilities
  • Travel between BGCLC sites and community partner locations as needed.
  • Collaborate with Program staff, leadership, and community partners to strengthen support services.
  • Contribute to Project Thrive and other organizational initiatives.
  • Perform other duties as assigned.
Qualifications Required
  • Associate's or Bachelor's degree in Human Services, Social Work, Public Health, Education, Psychology, or a related field; or an equivalent combination of education and experience.
  • At least two years of experience in family services, case management, resources navigation, care coordination, or referral services.
  • Knowledge of community resources, healthcare systems, behavioral health services, and public benefits available within Lake County.
  • Strong relationship-building, organization, communication, and problem-solving skills.
  • Experience maintaining accurate records using case management or CRM systems.
  • Understanding of mandated reporter responsibilities and willingness to complete required training.
  • Valid driver's license, reliable transportation, and the ability to travel between Club locations.
  • Ability to complete all required background checks and employment screenings.
Preferred
  • Bilingual (English/Spanish).
  • Experience working within Waukegan, North Chicago, or neighboring communities.
  • Youth Mental Health First Aid certification or willingness to obtain within 90 days.
  • Experience supporting grant-funded programs and collecting outcome data.
  • Familiarity with benefits enrollment and resource systems such as SNAP, Medicaid, and All Kids.
